Preface: This is a practice exam question to help prepare (intern Australian pharmacist) for part 4 of their oral exam. Other health professional students may find it useful and are free to participate. The case will have multiple problems but will have one main issue you must work out.

Case

A 65 year old man comes into your pharmacy and hands you this prescription.

For the purpose of practice exam, today’s date is 29/7/18 and all legal requirements have been met.

Bob has recently come back from an overseas trip. The Nexium has been prescribed because he thinks he might have upset his stomach by eating too many exotic and spicy foods.

Your dispensing records show the following:

For the purpose of this case, you’ve asked and Bob and gathered the following information:

  • He has been on Zanidip and Lipitor for a number of years.
  • Doxy was for his overseas trip. He went to some areas that may have malaria.
  • Actonel fairly new. First had on the 6/6/18.
  • Supplements: fish oil which he takes 1 capsule every day. And a calcium supplement which he takes every day except the day he takes Actonel.

There is more than enough information to find multiple potential problems. Though you may need to ask some questions to find the one we are looking for.
